2010 Leslie O. Fulton Fellowship
Application
$5,000 to be awarded
PURPOSE
This Fellowship is directed to an established theater artist for personal growth, enrichment, or renewal. The purpose of the award is to help artists of high merit sustain or enhance their creative process. It is not intended to support a staff position at an established company or provide operational support. Rather, we wish to honor an established artist with the time and funds to explore something beyond everyday responsibilities.

PROCESS
The Fellowship recipient will be chosen through an application process determined by a Selection Panel appointed by the PCTG Endowment Fund Committee.
ELIGIBILITY CRITERIA
Applicant must: live in the Portland Metropolitan area (includes SW Washington) in terms of official residence or professional activity be a professional artist in working practice for a minimum of five years. be recognized as a professional by his/her peers

EVALUATION CRITERIA
Applicant will be evaluated in the following areas: personal qualifications artistic excellence of proposal contribution to Portland theater financial information value of project to candidate’s personal growth feasibility of successful completion of project

APPLICATION PROCEDURE
1. Applicant must submit a complete application package with 8 full sets of materials. If you would like anything returned, you must include a self-addressed stamped envelope with sufficient first-class postage, not book rate, for return of any desired materials. To ensure return, these materials should be clearly identified.
2. Applicant must send the required 8 sets of the application package to:
PCTG Fellowships, PO Box 1181, Portland, Oregon 97207-1181.

3. Applications must be postmarked by April 16, 2010 to be accepted.
4. Application must be complete to be considered.
5. The Fellowship Selection Panel will choose the recipient.
6. The award will be made in June 2010 at the Drammy Awards Ceremony. All applicants will be notified by mail.

CONDITIONS OF AWARD
1. Funds must be used prior to May 30, 2011.
2. Credit must be given to the Portland Civic Theatre Guild in all promotional materials pertaining to work produced with support of the award, and submitted to the Guild for approval prior to use.
3. Recipient will submit a one page mid-year progress report by January 30, 2011, and a final report within 30 days of completion. Recipient will also make a brief oral presentation to the Guild membership.

Application Requirements:
These requirements must all be followed for application to be considered.
1. Complete application form. Please type or print neatly.
2. Answer each of the items below (1-4) on one single-sided sheet of white paper per item.
3. Provide on 8.5 x 11” white paper, single-sided: Resume and two letters of recommendation: One must be from a local theater company; the originals must be signed by the reference.
4. Supplementary material is optional, but limited to three single-sided 8.5 x 11” pages. This could be an additional reference, press clippings, illustrations, etc.
5. Make 8 copies of each application package: application form, answer sheets, resume, letters of recommendation, supplementary material (if any). Three-hole punch, assemble into sets with paper clips. Do not staple or put into individual folders. All pieces of application must be in the original packet. Items that come separately will not be accepted.
6. To submit a script, you must provide 8 copies of a one page synopsis of plot and one copy of the whole script. The synopsis must be three-hole punched and assembled into sets with paper clips. Do not send 8 complete scripts.
7. DO NOT send tapes, CDs or videos.
8. If you wish any materials returned, please clearly identify what is to be returned and include a self-addressed envelope stamped with sufficient first-class postage, not book rate, to cover the weight and cost of mailing.
9. Mail all application materials to PCTG Fellowships, PO Box 1181, Portland Oregon 97207-1181. Application must be postmarked by April 16, 2010 to be eligible

When answering the following, each answer should be typewritten on a single sheet of 8.5 x 11” white paper with margins of one inch on each side. Use black ink with a 10 or 12 point font. Number and restate the question at the top of each page, before your answer.
1. Explain your proposed project or objective.
2. How do you think this will advance your career?
3. Attach a one-page budget and explain how the funds will be used. If other grants are listed, please specify if actual or potential. Include grantor contact names and phone numbers. If you are depending on other grants that have not yet been awarded, suggest how you would proceed with this fellowship if these other potential grants do not materialize.
4. If your project is dependent upon the participation of other artists or institutions, please provide a brief statement of their support. Include contact names, phone numbers and email addresses.
5. Include signed permission to adapt another person’s materials, if appropriate.

All theatre companies in the Portland Metropolitan area (including SW Washington) are eligible to apply. Funds may be spent on capital items or production expenses. Funds may not be used for general operating expenses.
